By completing and signing this form, I hereby agree to the following:

1. I will abide by the Code of Conduct and Safety Guidelines at all times while using Mission Possible’s facility and participating in Mission Possible's event. I understand that failure to comply may lead to loss of privileges to access the facility.

2. I agree to assume full responsibility for any risks, injuries, damages, or losses, which I might incur as a result of accessing Mission Possible’s facility and participating in Mission Possible's event.

3. I, my heirs, and my legal representatives, knowingly, voluntarily, and expressly forever release and discharge Mission Possible, its staff, and its volunteers from any and all liability, claim, demand of action that I may have related to any personal injuries or any damages or losses of any of my personal property that may be related to my use of Mission Possible’s facility or participating in Mission Possible's event, including incidents that may be caused by the negligence of the released party.

I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
